Requesting Medical Information

Update

  • The surgery is putting a temporary pause on all requests for information from patients and third parties until Monday 5th January 2025. During this time, you will be unable to submit a request for medical information to the surgery and no requests will be processed during this time.
  • We will re-open our request process on Monday 5th January 2025 and patients will be able to submit after this date
 

If you are requesting medical information for university purposes, please go to the appropriate section below.

Our standard charge for completing a medical report or form (e.g.: for an insurance company) is £25.00. As part of this process, you will be asked to provide photo ID with Date of Birth shown (i.e.: passport, drivers licence, residence permit, etc.) as well as written authorisation if you are requesting this on behalf of a patient or proof of address if you are requesting on behalf of your child.

Please note we are unable to provide letters from the GP. We have alternative methods, which include copies of recent consultations, results or brief patient summaries that may be helpful. This service is free of charge and you will still need to go through the same process as stated above.

Any request for medical information will be completed within 30 days. If the request is likely to take longer than this, our Admin Team will be in contact with you to discuss this further.

Student Requests

If you are a university student and need to make an application for compelling personal reasons (CPR) or Disabled Students Allowance (DSA) / Student Finance, you will need to fill out a Student Medical Evidence Request Form by clicking on the link below.

We will use this form to generate a medical report for which there will be a fee of £25.00 payable on completion of the request. Students in severe financial hardship can apply for financial support from the University Support Grant to cover these costs. If we are unable to see that you are eligible for DSA, a member of the Admin Team will contact you to discuss this further.

Camp America Forms

If you are in the process of applying for Camp America and require the doctor to complete a medical form, this no longer needs to be done through an appointment at the surgery.

Please click on the relevant option to complete a request, attach the forms with your completed section to the request and make a £55.00 payment for completion of this.

Lasting Power of Attorney (LPA)

Some GP practices may offer to act as a Certificate Provider for a Lasting Power of Attorney (often with a charge) but at this time, our own policy is not to offer such a service. At this present time, it is imperative we prioritise patient care and as such, sadly we do not have capacity to undertake this kind of work.

You will need to have your LPA signed by a Certificate Provider. This is someone who confirms that the LPA has been understood and a person has not been put under any pressure to sign it. This certificate provider must be someone you know well or a professional person such as a doctor, social worker or solicitor.
